Patient Assistance

Applying For Financial Assistance: If you are a leukemia patient who would like to be considered for limited financial aid, send an email describing your situation and including the patient's name, home address, phone number and name and address of the hospital where treatment is being received to: stuartmosher@aol.com, or mail it to: The Shannon Mosher Memorial Foundation, Inc. at 3589 Rock Elm Court, Auburn, GA  30011. Our Patient Assistance program provides aid in form of reimbursements for certain paid expenses not covered by your insurance provider.  If tentatively qualified, a financial grant application form will be sent to you for completion. A parent or other involved family member or friend, acting on behalf of a leukemia patient, may also assist in providing the appropriate information to the Foundation.

GOLF BENEFIT RESULTS - May 3, 2008:
We had a tremendous response to the first charity golf tournament that was held to benefit the Foundation's Patient Assistance program, held in Ocala, FL on May 3, 2008, netting $3,726 that will be used to provide some financial relief to several leukemia patients. Teresa and I were pleased that we were able to be there, and it was especially rewarding to have one of the local patients from the Ocala area, 8-year old Angela Page, who we have been assisting since 2004, there with her parents, Viodelda and Myron. The corporate sponsors and golfers were also able to meet this family, and they were very happy to have a personal picture of what their involvement would mean to Angela and her family and other leukemia patients in need of financial assistance. We hope to be able to continue providing financial assistance to many deserving leukemia patients, as long as there is a need and we have funds available. Thank you to all the golfers, sponsors and others, particularly Danielle and Palmer Pendleton, who helped to make this event happen and to cause it to be such a great success!! Below are some more details about this event.
 
CHARITY GOLF EVENT:
On May 3, 2008, the annual Performance Elastomers, Inc. Golf Scramble, will be held at the Silver Springs Shores Golf & Country Club in Ocala, FL. We are blessed that this year our Foundation will be the beneficiary of the charitable net proceeds resulting from this event. Special thanks go to our friends, Palmer Pendleton, from Class 1 and his wife, Danielle, who works with Regions Bank, who were both very instrumental in getting the Foundation approved for this event.
 
All of the net proceeds received by the Foundation from this event will go into our General Fund, and 100% of those proceeds will then be redistributed to leukemia patients and their families as reimbursements of expenses not covered by their insurance provider. After the death of our daughter, Shannon, from leukemia on May 20, 2000, we fulfilled her heartfelt desire to do something to help others who face this disease by establishing this 501(c)(3) non-profit Foundation in her memory. Our primary efforts are directed to providing financial assistance to leukemia patients for expenses not covered by insurance, and to informing and encouraging others to become registered on the national registries as bone marrow/stem cell donors.  Since the Foundation sent its first reimbursement check to a leukemia patient six and a half years ago, the Foundation has had the honor and privilege of providing tangible financial assistance to 32 leukemia patients and their families. 
 
With the generous donations received from family, friends and others, the Foundation has provided 135 individual grants to these patients totaling $24,466.62. As a relatively small Foundation with limited resources, our Board has approved $1,000 as the maximum that we're able to provide any one patient during a 12-month period.  At some point, we would like to be able to increase the maximum that our patients can receive because, in many cases, the need is so much greater than what we can fulfill. 
 
Teresa and I receive no compensation for our service to this cause and we cover the cost of most of the administrative expenses.  This is just something that we've been called to do, and we feel blessed to have the opportunity to be able to fulfill Shannon's desire related to these needs. When Shannon was in desperate need of a bone marrow donor during her second remission, we held a bone marrow/stem cell drive at our church on 2/6/00, and 420 new volunteer donors were registered.  The Foundation has since co-hosted and/or assisted at 11 drives, during which 948 other new donors have been added to the national registries.
 
As 2007 draws to a close, we only have about $300 remaining in our account for Patient Assistance.  All tax-deductible contributions received go into our General Fund, unless otherwise designated, and 100% of those funds are redirected back to leukemia patients as reimbursement for many of the expense not met by their insurance provider. Would you please consider a year-end donation to the Foundation and help us continue to meet the needs of patients and families?

May 11, 2004 - Three-year (less a month) Milestone
Three years ago (less a month) today, the Foundation was able to send its first check to a leukemia patient approved for financial aid as part of our Patient Assistance program. Including that first patient, the Foundation has had the honor and privilege of providing tangible financial assistance to 24 leukemia patients and their families. With the generous financial support and donations from family, friends and others, the Foundation has provided 84 individual grants to these 24 patients totaling $14,997.04.

June 11, 2002 - One-Year Milestone
One year ago today, the Foundation was able to send its first check to a leukemia patient approved for financial aid as part of our Patient Assistance program. A total of three checks totaling $750 over a 3-month period were sent to this patient who was having to battle the many problems inherent after going through a bone marrow transplant. These reimbursements were for some expenses incurred by the patient and his family not covered by insurance.
 
Including that first patient approved a year ago, the Foundation has approved a total of 15 patients for Patient Assistance (and there are two pending) and has been able to issue 43 checks totaling over $6,800 to 13 of them. We’ve been able to provide this financial aid only because of the generous contributions received from many people, such as:  donations received from the family and friends of a leukemia patient who passed away last year as memorial gifts in his honor; in January this year, a generous gift from someone in Wisconsin who apparently just heard about our Foundation and felt led to make a contribution; and gifts from Shannon’s family and friends, some of whom have selected the Foundation as the agency to receive their contributions through the annual United Way drive at Bank of America. There have been others, and we feel truly blessed when folks who want to make a

difference in the lives of leukemia patients they don’t even know support the Foundation.
 
As a relatively new Foundation with limited resources, $1,000 is the maximum that we’re able to provide any one patient during a 12-month period and, except for two such situations the past year where we’ve reached this maximum, to date we have not had to reject a specific financial request from a leukemia patient. We would like to keep that record in tact, and at some point be able to increase the maximum that any one patient can receive. With your help, these can be reasonable objectives. It’s our desire to honor what Shannon so much had hoped to be able to do had she survived. Teresa and I receive no compensation for our service to this cause; it’s just something we've been called to do and we feel blessed to have the opportunity to be able to fulfill Shannon's desires related to these needs.
